Registration is available up to 14 before the first vaccine date and up to 21 days before the second vaccine date.

Parent/Guardian Permission Process:

• For students ages 12-15, a parent/guardian needs to accompany the child to the vaccine appointment.
• For students ages 16 to 17, minors can receive verbal confirmation via a phone call at the time of the appointment.
